We accept all major insurance plans and we will be happy to file your claim for you. Insurance cards are to be presented to the front desk at each visit. lf you do not have your insurance card, you will be responsible for payment of the charges the day of the physician visit or we will be happy to reschedule your appointment for another time. Presenting an insuance card at each physician visit is required by the terms of your insurance contract.
Deductibles and Co-Insurances
Deductibles and Co-Insurance are a contracted agreement that you have with your insurance company. You have agreed, by the terms of your insurance contract, to pay them.
Surgery patients will be responsible for paying their deductibles and/or co-insurance prior to the surgery being performed. These are usually paid at your pre-op visit You will be sent a letter, or personally called, informing you of the amount due.
Patients who do not have a designated copay will be responsible for paying either the deductible and/or co-insurance at the time services are rendered. These amounts will be calculated at the time you checkout and determined by your charges for the day.
Patients with no insurance coverage are responsible for payment the day of the physician visit.
CoPays (as stated on your insurance card) are a contracted agreement that you have with your insurance company. You have agreed, by the terms of your insurance contract, to pay them. CoPays are to be paid each time you come to the physicians office, for any service.
A certain amount of time is held specifically for you when you schedule an appointment. As a courtesy to our providers and other patients also in need of medical attention, we require a 24-hour notice if you need to cancel or reschedule your appointment for any reason. Otherwise, a $25.00 fee will be assessed to your account.
